Welcome to The Institute Rockwall

Thank you for your interest in becoming a student at The Institute Rockwall. We are excited to help you begin your journey toward a rewarding career in the beauty and barbering industry. This application is the first step in determining your eligibility for admission and gathering the information required for enrollment, state licensure, and participation in our partnership programs with Lamar State College Orange.

To complete this application, please have the following information and documents available:

• Government-issued Driver License or State Identification Card (if applicable)

• Social Security Number

• A recent passport-style or school photo (head and shoulders)

• Information about your educational history, including high school, GED, college, and any previous career or technical education

• Information regarding any previous cosmetology, barber, esthetics, manicuring, or eyelash extension training, including schools attended and hours completed

• Information regarding any professional licenses you currently hold or have previously held

• Information about any criminal convictions or disciplinary actions that may require review under Texas Department of Licensing and Regulation (TDLR) requirements. A criminal history does not automatically prevent admission or licensure, but additional review may be necessary.

• Emergency contact information

• Parent or guardian information (if you are under 18 years of age)

Please complete all sections of this application accurately and completely. Providing false or incomplete information may delay the admissions process or affect your eligibility for enrollment. If you are seeking a program that qualifies for financial aid, please choose from one of our two associate degree options - Associate Degree - Texas Class A Barber or Associate Degree - Texas Cosmetology Operator. Personal Identification Information

Effective May 1, 2026, the Texas Department of Licensing and Regulation (TDLR) requires all cosmetology applicants and licensees to prove lawful presence in the United States. U.S. citizens generally satisfy this automatically via their Social Security Number, while lawful noncitizens must provide approved residency or visa documentation.

The list of acceptable documents is available on TDLR’s Verification of Lawful Presence webpage. Applicants who do not have a Social Security number (SSN), are adding an SSN, or correcting an SSN must complete the required form and provide supporting documentation from the list of acceptable documents.

If you do not have a social security number, please reach out to us and schedule a face to face appointment to complete your application for admission. We will assist you with required documentation. info@nstituterockwall.com.

By signing below, I acknowledge that I have read and understand the following:

• The $50.00 Application Fee is non-refundable under all circumstances.

• The application fee includes the required $25.00 Texas Department of Licensing and Regulation (TDLR) Student Practitioner Permit Application Fee.

• I have met with an Admissions Advisor and understand the admissions process, program requirements, and enrollment expectations.

• I understand that my space in an enrollment cohort is not reserved until all of the following have been completed:

  • Submission of this completed Application for Admission;

  • Payment of the non-refundable $50.00 application fee; and

  • Submission of my Lamar State College Orange Application for Admission.

• I understand that acceptance of my application does not guarantee enrollment and is contingent upon meeting all admissions requirements and space availability.

• I understand that my required down payment, or payment in accordance with my approved payment arrangement, must be received before I will be permitted to begin classes or participate in training at The Institute Rockwall.
